New Criteria for Prediction and the Assessment of Effective Therapy of Ulcerative Colitis in Children
The aim of the investigation is to determine the structural criteria of ulcerative colitis prediction and the efficacy of therapy based on the analysis of cell-tissue relations in intestinal mucosa.
Materials and Methods. In order to reveal the prognostic indices of thedisease there have been retrospectively studied the bioptates of 26 children with recurrent ulcerative colitis and 25 children — with continuously recurrent ulcerative colitis. To determine the morphological criteria reflecting the need for surgery, the bioptates of 26 children operated for ulcerative colitis have been retrospectively studied.
Results. The main factor of favourable prognosis for ulcerative colitis is the absence of changes of tissue relations in the organ system. Continuously recurrent ulcerative colitis is characterized by the appearance of new structural features on tissue level and in the system of two interacting tissues of organ level — epithelial and areolar tissue of lamina propria. If the disease progresses involving the neighbouring organs or the entire wall of the large intestine, an aggressive variant of pathomorphosis can be diagnosed.
Conclusion. Structural indices of cell and tissue elements composing large intestine mucosa are responsible for the predictability of ulcerative colitis course in children. The indices are quite new features of regeneration histogenesis of cellular, tissue and inter-tissue levels in epithelial layer, lamina propria, and significant disorder of inter-tissue relations in organ system.
