Surgical Treatment of Autoimmune Thyroiditis
The aim of the investigation is to prove the choice of optimal extent of operative measure based on the study of short- and long-term results of surgical treatment of patients with autoimmune thyroiditis.
Materials and Methods. 19 women with autoimmune thyroiditis aged 39—56 have been included into the prospective controlled study, the duration of the disease being from 1 to 9 years. The investigation results have been assessed based on a follow-up, the analysis of thyroid status indexes, ultrasound findings and patients’ questioning.
Conclusion. To prevent complications development in compression syndrome in patients with autoimmune thyroiditis, it is reasonable to perform subtotal thyroidectomy leaving thyroid tissue in tracheoesophageal sulcus on either side of the trachea. Intrafascial thyroidectomy is the operation of choice if there are nodes against the background of chronic autoimmune changes in thyroid tissue. Extrafascial thyroidectomy with central lymphatic dissection is to be performed if thyroid carcinoma is reasonably suspected.
