External Transintestinal Wirsungostomy is a Procedure of Pancreatojejunal Anastomosis Incompetence Prevention
The aim of the investigation is to optimize a reconstructive stage of pancreatoduodenal resection (PDR) by forming an external transintestinal wirsungostome.
Materials and Methods. The PDR results in 82 patients treated in A.I. Kozhevnickov Surgical Clinic of Nizhny Novgorod Regional Clinical Hospital named after N.A. Semashko have been analyzed. In the first group PDR have been completed with the pancreatodigestive anastomoses and in the second group — with an external wirsungostome. A pancreatic juice outflow in the third group has been restored by an external transintestinal pancreatojejunostomy.
Results. Performing PDR by means of a proposed technique — by a single-step applying an external transintestinal wirsungostome enables to avoid the second stage of an operation. The incretory and exocrious functions of the pancreas are preserved. Diabetes mellitus and enzymatic insufficiency do not develop. The period of treatment is reduced from 5—6 months to 4 months. It is possible to control a pancreas stump state according to the pancreatic juice quantitative and qualitative indices.
